Payment Options

Payment is expected at the time services are rendered for medical and cosmetic services and products. For your convenience we accept cash, check, major credit or debit cards, and CareCredit (restrictions apply). Payment plans through automated bank, or credit card drafts are also available on medical visits.

Cancellation, Late Arrival and ‘No-Show’ Policy

We require 24 hours cancelation notice for general/medical appointments. If your appointment is cancelled less than 24 hours prior to your appointment time or if you fail to show up to your appointment all together, you will be charged a $35 fee. Late arrivals may require rescheduling. There will not be a cancellation fee unless you arrive more than 30 minutes after your scheduled appointment. Remembering your appointment is ultimately your responsibility. AS a courtesy we use an automated reminder system that may alert you 48 hours prior to your scheduled appointment. Please keep us informed of your current telephone number. This practice reserves the right to discharge patients who have cancelled with less than 24 hours’ notice or ‘no-showed’ for their appointments in excess of 3 times.

Patient Registration Process

New patients are asked to complete a new patient registration package at the time of their initial appointment. For your convenience this package can be downloaded here and completed prior to your visit or you may arrive 15 minutes early for your appointment to allow time to complete them in the office. Please bring a photo ID and an insurance card to your appointment.
